(Revised program effective October 1, 2007)
This life and AD&D insurance program along with MEDEX is provided as a benefit of membership to all regular SATA members who are citizens of the U.S.A. or Canada and furnish SATA with a date of birth. The premium for the insurance and MEDEX is paid for by SATA.
Coverage for new regular members begins on the first day of the second month following the date their application is approved and processed. For example a person joining on November 16 will have coverage on January 1.
Life and AD&D
Coverage for Regular Members is as follows:

MEDEX

If you would like to receive a copy of the certificate of insurance for your personal records, please click here. This certificate is not individualized, as the policy is issued in the name of the association.
Beneficiary Forms
